University of Utah Health Care takes the privacy of your health information very seriously. We promise that we will protect that information to the best of our ability through the training of our faculty, staff, and volunteers to use or disclose patient data with the utmost caution and consideration. Notice of Privacy Practices - Your Rights (HIPAA)
Every patient we serve is provided a Notice of Privacy Practices (PDF) the first time they come to us for health services. This document is the centerpiece of our promise to you. It serves two important functions:
- It tells you how we use and/or disclose your information to make sure you get the best treatment, with as little inconvenience as possible, for you.
- It advises you, the patient, of your privacy rights in the control of your own health information.
It is available in both paper and electronic form and in Spanish.
